Consent Forms

NOTE:  Pick the consent form from the ones below according to the needs of your study. All the consent forms must be modified to briefly describe the study. Do not disclose your manipulation/conditions because participants will fill it before the interaction.

- User studies WITHOUT video/audio recording must use this consent form.- User studies WITH video/audio recordings must use this concent form.
- User studies with participants that have less than 18 years old must use this consent form, which should be signed by the parents. 

Regarding material you will need from GAIPS (e.g. robots, cameras...)

- You need to set a date and time with me or Luís Silva (GAIPS lab admin, email: luis.silva@gaips.inesc-id.pt) to grab the material and sign the responsibility form.-  Some people will need the same resources, and therefore we have created a google spreadsheet to organize the needs of every group. One person per group should answer this spreadsheet. As soon as I find some inconsistencies in your requests, we will try to manage them. LINK TO SPREADSHEET
