)There are three types of consent forms.

NOTE: Pick the consent form from the ones provided according to the needs of your study.
All the consent forms must be modified to briefly describe the study.
Do not disclose your manipulation/conditions because participants will fill it before the interaction.
- User studies WITHOUT video/audio recording must use the general consent form (ConsentForm.doc).
- User studies WITH video/audio recordings must use the consent form that states the use of video recordings (ConsentFormVideo.docx).
- User studies with participants that have less than 18 years old must use consent forms for minors (children) which should be signed by the parents (ConsentFormChildren.docx).
